CONFERENCE POLICY & FRAMEWORK
This policy establishes the requirements, responsibilities, and expectations for all quadball conferences officially recognized by USQ.

CONFERENCE EVALUATION
Submission form for conferences interested in joining USQ on an officials basis. The USQ Conference Policy and Framework Policy should be reviewed before filling out this form.

EVENT ATTENDEE FEEDBACK FORM

Use this form to provide feedback on any community event you attended this year. You can review events you spectated, officiated, and/or competed in.

EVENTS FINE POLICY
Events sanctioned by USQ must meet specific requirements and adhere to established deadlines. Inability to meet the requirements of the various types of USQ tournaments may result in a fine.

JOINT TEAM DEVELOPMENT APPLICATION (PENDING ADDITION OF A/B TEAMS)

This joint team development program is intended to help new teams develop. The joint team program has been expanded to allow schools with previously official teams to combine, provided they have evidenced need

MEMBER CODE OF CONDUCT & UNLAWFUL HARASSMENT

The Member Code of Conduct requires all participants to uphold sportsmanship, respect, and inclusivity while prohibiting harassment, discrimination, substance abuse, and unsportsmanlike behavior, with violations subject to disciplinary action

STAY AND PLAY POLICY

The USQ Stay and Play policy (hotel requirements) applies to all USQ-run tournaments, including national qualifiers and US Quadball Cup

GAMEPLAY POLICIES & WAIVERS

CLUB COMPETITIVE DIVISION OPT-OUT FORM
Club teams that wish for a less competitive division can opt out of the club competitive division prior to USQ-hosted national qualifiers

COACH POLICY: EMERGENCY EXCEPTION
Report any instances where a team's certified coach needed to leave the event site due to a medical or personal emergency that arises during an event

EQUIPMENT POLICY
Specifies the standards and requirements for all player gear and balls used in official play, including safety guidelines and approved equipment types

EXCEPTIONAL CIRCUMSTANCE: GAMEPLAY REQUIREMENTS EXEMPTION REQUEST
A team must fill out this form if they feel there has been exceptional circumstance(s) that has caused the team to be unable fulfill gameplay requirements for the season

FAIR PLAY AND UNAUTHORIZED INTERFERENCE POLICY
Defines expectations for sportsmanship, respectful behavior, and integrity to maintain a safe and welcoming playing environment and prohibits unauthorized individuals from interfering with games or officiating crews in order to protect match integrity and ensure a safe, respectful environment for referees

FORFEIT AND CONCESSION POLICIES
Explains the conditions that constitute a forfeit and how forfeits are recorded in official standings and outlines the circumstances under which a team may concede a match and the process for doing so

GAME APPEAL POLICY
Provides a process for teams or players to formally appeal the outcome of a game or a ruling made during competition

INCLEMENT WEATHER POLICY
This policy will be used at all USQ-hosted events to determine actions based on weather conditions.

PLAYER DISCIPLINARY POLICY
Describes procedures and consequences for player misconduct or violations of league rules

PLAYER EQUIPMENT WAIVER
Submit equipment such as concussion headbands, knee braces, etc. for approval via this form

PLAYER TRANSFER POLICY
Regulates how and when players may switch teams or rosters during the competitive season. The league-wide transfer deadline is Sunday, April 12 at 11:59 pm ET for 2026

TIEBREAKER POLICY
Defines the rules and methods for resolving ties in standings, matches, and tournament rankings

GENDER MAXIMUM POLICY
Ensures gender inclusivity by limiting the number of players of the same gender on the field at one time to promote diversity and equity in gameplay

USQ NATIONAL QUALIFIER REFREE CREW FORM

All teams attending a USQ National Qualifier are required to submit the team's referee crew

USQ NATIONAL QUALIFIER REGISTRATION (COMING SOON)

All teams are required to register for qualifiers to collect information and confirm attendance at the event. This registration is due by January 15
