HEAD REFEREE CERTIFICATION
1. Become LAR certified
In order to be considered a head referee candidate, you must be currently Lead Assistant Referee certified.
2. Register with USQ as an official
Whether you are a player of non-player, you must also have a USQ membership. You may sign up either before or after completing your tests if you are an HR, LAR, or flag runner. All officials memberships will expire on June 30.
3. DEI Training
Take these courses and screenshot your completion certificates. Upload them to this form.
4. W9 to USQ
To be paid for officiating at a USQ-hosted event, please send a W9 form to finance@usquadball.org.
5. Field Test
Take and pass a field test. A field test is the act of head refereeing a live game while being proctored. You can sign up for a field test using this form.

RECERTIFICATION
All officials looking to recertify must complete these three steps. Other steps are dependent upon tier.

Seasonal Game Requirement
Fully certified Head Referees of all tiers must officiate or proctor at least seven games per quadball season (July 1 - June 30).
If, by June 30, a fully-certified Head Referee has not finished their game requirements, their field test certification expiration date will automatically change to the end of that calendar year (December 31).
To remain fully certified, the Head Referee must get a new Field Test Certification with an updated tier designation. This updated tier can be the same as their old tier. Exceptions to this policy can be considered on a case-by-case basis
